Sooner or later, someone asks who changed a record. A price went down, a customer disappeared, an invoice flipped to paid. In a custom system without an audit trail, the honest answer is that nobody knows. Adding one later recovers nothing from the past.
An audit trail is a separate, append-only record of meaningful actions. Each entry stores who acted, what they did, which record it touched, when it happened, and the values before and after. Ordinary users can read it but never edit it. Even administrators should not be able to quietly remove entries.
The first design choice is scope. Logging every field on every table creates a huge, noisy store that no one reads. Logging too little misses the one event that matters.
